⚠️ This is a generic template. Customize for your equipment, site, and standard.

What to Do With Failures

Any item marked "Fail" must be resolved before the equipment is used or work continues. Document the failure with photos, notify the supervisor, create a CAPA in HSEKit, and follow up to verify the fix is effective. Repeat failures indicate a systemic issue that should trigger a risk assessment review.

Replace paper with HSEKit. Mobile-first inspections, photo evidence, auto-CAPA, trend analytics, scheduled recurring inspections.
